Thought the food was pretty good. Service was a little confused and it took like an hour for each course, which is not bad, just unexpected. Liked my lasagna and chopped salad. The tiramisu was awesome. Nice little spot on Montana.

Very welcoming atmosphere and service. They have nearly 100 types of cheeses to try. The portion sizes for the entry were a bit small for the cost. I would still visit again and try more of their cheeses.

5+ for everything! Amazing food and staff! It’s super cute and everything is just amazing! Started off with some really good cheeses! The selections are great and the staff is all super helpful. Got the bordeux wine and omg sooooo delicious! For dinner we got the truffle fettuccine and risotto which were perfectly made. I don’t think you can go wrong with any dish here.

We are not fancy people. But this place was the perfect “neighborhood fancy” that felt special yet still comfortable, and we just adored it. I took my partner here to celebrate his birthday. Everyone was so lovely, and the food was incredible. Our favorite dish was a simple cheese pizza. I think, in total, we only spent around $100. When friends and family visit, we have a short list of local places we recommend. Forma is at the top of that list.

Recently, my family and I had dinner at Forma in Santa Monica and it was a delightful experience. We specifically went to try their “Dalla Forma” pastas, which are mixed in a cheese wheel. The pasta was noticeably fresh, and the method of mixing it in the cheese wheel added a wonderful depth of flavor. In addition to the pasta, we tried the Margherita pizza, which was impressively thin and crisp, just the way we like it. The Burrata served with beets was a great combination, providing a pleasing contrast in flavors. But the star of the night was their Tortellini, our absolute favorite dish. Each dish was crafted with care and the quality of the ingredients shone through. Forma left an excellent impression on us and we highly recommend it for anyone looking for a unique dining experience in Santa Monica. We can’t wait to return!

Cheese-focused Italian spot on Montana Ave. Came here on a Sunday around noon and made a reservation online beforehand to celebrate my bday with family. Indoor and outdoor seating is available. Metered street parking, although free residential parking (most were for 2 hours, exempting permits) is available. Credit cards accepted. Kid-friendly. Single occupant restrooms available in the back. The food was all delicious. They make the pastas in the cheese wheel behind a plastic barrier so you can still watch, but they don’t do it at your table. We got an order of burnt spaghetti assassina, which is only slightly burnt, which lends a nice toasty flavor the the spaghetti. It’s good for four people. The cacio e pepe is a popular pasta and for good reason - it’s peppery and salty (from the Parmesan). The mushroom pasta was similarly quite delicious; very creamy and rich. The fettucine bolognese had good flavor and kept relatively well for leftovers. My dad got a Sicilian flatbread (it’s vegan) and it was still filling. For dessert, we got the gianduja semifreddo, which our waiter claimed was his favorite. It’s light yet still chocolately - I highly enjoyed it. Overall, it’s on the pricier side, but it’s a delicious meal with big portions and friendly service. You’ll have an enjoyable time here. Bon apetito!

Amazing happy hour! Such a friendly atmosphere and staff. Feels like a neighborhood spot. Will absolutely be returning.

What a great Italian restaurant! Big cheese selection, delicious pasta and risotto, some of which are made in a cheese wheel. They also have quite an extensive wine card and even some Bavarian beer. Lobed it!

A good time had by all. Nice place to visit, talk and have a meal. We ate inside and enjoyed the low ceilings, which allowed us to talk in our normal voices, a unique experience for many of us in today's high ceiling restaurants. The food is very rich, as everything has a tinge of cheese.

Delicious!! First time coming here and had a good experience. Noodles were so good, and buttery and creamy. The cheese board was really fun. We would definitely come back! Staff was friendly, resturant was clean and nice. Bathrooms were nice also.
